That was my impression from reading the logs, particularly the "ata"
messages.

The logs seem to indicate a disk-related issue:
> ata4.00: device reported invalid CH sector 0
CH, isn't that a cylinder / head reference?
Since it's also problematic booting from live DVD, I'd look at the SATA
card that the disk(s) connect to.
Maybe try a live USB stick - that would use a different bus. If it
works, it lends confidence to a disk / SATA card issue.
